School Health, Physical, and Immunization Forms
All new students to Rantoul Township High School must complete these forms for registration. This includes incoming 9th graders and any students entering the school for the first time in grades 10-12. In addition, 12th grade students are required to meet the Health requirements listed in the document under 12th grade Health Requirements.

Allergy Health History
This document is used to report Allergy Health History. It needs to be completed even if a student does not have a current medical condition, allergy, or special need at the time of registration.

Annual IHSA Sport Physical Information and Forms
9th graders are required to have an initial physical on file upon registration.
After the initial physical in the 9th grade year, student athletes in 10th, 11th, and 12th grade must receive a physical that follows the IHSA guideliens for Sports Physicals.

Dental Exam Forms and Waivers
Each incoming student is requried to provide record of a dental examination. This is a new requirement that started in the 2021-2022 school year.

Emergency Form
This form informs the school of who should be contacted in the case of emergency.

Eye Exam Forms and Waivers
An eye exam is required the first time a student enrolls in an Illinois school.

Immunization Information
This document lists the immunization requirements for students.

Medication at School
This form must be completed when a student needs prescription medication and NON-prescription medication during school hours.

New Student Physical Form (9th Graders and New to District Students)
All students new to the District (9th graders and any student attending Rantoul Township High School for the first time) must have this physical form completed by a doctor prior to the first day of school.

Religious Exemption Form
Parents or legal gaurdians who are requesting a religious exemption to immunizations or examinations must use this form for students entering ninth grade.
